miR-181d is a microRNA being investigated as a therapeutic strategy for glioblastoma. It acts as a master regulator that simultaneously inhibits homologous recombination (HR) and base excision repair (BER) pathways by targeting RAD51 and O-6-methylguanine-DNA methyltransferase (MGMT). In glioblastoma, miR-181d levels are often decreased following temozolomide treatment, leading to the derepression of MGMT and RAD51, which confers resistance to chemotherapy and cross-resistance to ionizing radiation. Exogenous administration of miR-181d has been shown in preclinical models to sensitize glioblastoma cells to DNA-damaging agents by suppressing these repair mechanisms.
