Mirvetuximab soravtansine + sacituzumab govitecan is an investigational combination therapy comprising two distinct antibody-drug conjugates (ADCs). Mirvetuximab soravtansine (Elahere) is a folate receptor alpha (FRα)-targeted ADC that utilizes a cleavable disulfide linker to attach the maytansinoid DM4, a potent microtubule-disrupting agent. Sacituzumab govitecan (Trodelvy) targets the trophoblast cell-surface antigen 2 (Trop-2) and delivers SN-38, the active metabolite of irinotecan and a topoisomerase I inhibitor, via a hydrolyzable linker. This combination is primarily being evaluated for patients with recurrent platinum-resistant ovarian, fallopian tube, or primary peritoneal cancers. The rationale for the combination is to leverage the high expression of both FRα and Trop-2 in these malignancies and to provide synergistic anti-tumor activity through non-overlapping mechanisms of action and distinct cytotoxic payloads.
