This combination therapy consists of mirvetuximab soravtansine-gynx and olaparib. Mirvetuximab soravtansine-gynx (Elahere) is a first-in-class antibody-drug conjugate (ADC) targeting folate receptor alpha (FRα), which is highly expressed in many epithelial ovarian cancers. It comprises an FRα-binding antibody linked to the potent microtubule-disrupting agent DM4. Olaparib (Lynparza) is a small molecule inhibitor of poly (ADP-ribose) polymerase (PARP) enzymes, including PARP1, PARP2, and PARP3, which play a critical role in DNA damage repair. The combination is being investigated by the University of Colorado, Denver, as a maintenance therapy for patients with recurrent platinum-sensitive ovarian, fallopian tube, or primary peritoneal cancer. The therapeutic rationale involves the synergistic effect of targeted cytotoxic delivery via the ADC and the inhibition of DNA repair mechanisms by the PARP inhibitor, potentially overcoming resistance and improving outcomes in FRα-expressing tumors.
