Mirzotamab clezutoclax is a first-in-class antibody-drug conjugate (ADC) developed for the treatment of advanced solid tumors. It consists of an anti–B7-H3 (CD276) monoclonal antibody, a solubilizing linker, and a BCL-XL inhibitor payload. The drug specifically targets the immunoregulatory protein B7-H3 on tumor cells and delivers the BCL-XL inhibitor directly to these cells via the ADC mechanism. This targeted approach aims to inhibit tumor growth by inducing apoptosis in cancer cells that overexpress B7-H3 while minimizing systemic toxicity associated with direct BCL-XL inhibition. Preclinical studies have shown synergy with other anticancer agents, and clinical trials are ongoing in relapsed/refractory solid tumors including small cell lung cancer (SCLC), non-small cell lung cancer (NSCLC), and hormone receptor-positive breast cancer.
