Misitatug blivedotin (RC88) is a first-in-class antibody-drug conjugate (ADC) designed for the treatment of advanced solid tumors expressing mesothelin. It consists of a recombinant humanized anti-mesothelin monoclonal antibody linked via a cleavable linker to monomethyl auristatin E (MMAE), a potent microtubule-disrupting agent. Upon binding to mesothelin-expressing tumor cells, the ADC is internalized and MMAE is released intracellularly, leading to cell cycle arrest and apoptosis. Mesothelin is overexpressed in several solid tumors—including ovarian cancer, fallopian tube cancer, peritoneal cancer, mesothelioma, pancreatic cancer, bile duct carcinoma, and lung adenocarcinoma—while its expression in normal tissues is limited. RC88 has demonstrated encouraging efficacy and manageable safety in early-phase clinical trials for patients with platinum-resistant or refractory disease[1][2][4][8].
