MIST-B04 is a novel liquid formulation of botulinum toxin type A (BoNT/A) currently in clinical development by Mist Biopharma. Unlike traditional botulinum toxin products that are supplied as a lyophilized powder requiring reconstitution with saline prior to administration, MIST-B04 is a ready-to-use aqueous solution. This formulation is designed to enhance clinical safety and convenience by eliminating potential errors in dilution and reducing the risk of microbial contamination during preparation. Mechanistically, the toxin enters peripheral cholinergic nerve terminals by binding to synaptic vesicle glycoprotein 2 (SV2) and subsequently cleaves the synaptosomal-associated protein 25 (SNAP-25). This action disrupts the SNARE protein complex, effectively blocking the release of the neurotransmitter acetylcholine into the neuromuscular junction and resulting in temporary localized muscle paralysis. MIST-B04 is primarily being investigated for aesthetic indications, specifically the treatment of moderate-to-severe glabellar (frown) lines, and is also being explored for therapeutic applications such as muscle spasticity.
