Overview

Mitiglinide + gemfibrozil is a combination of two pharmaceuticals: mitiglinide, a rapid-acting insulin secretagogue (glinide class) used as an oral antidiabetic agent, and gemfibrozil, a fibric acid derivative (fibrate) utilized for its lipid-lowering effects, primarily in the management of hypertriglyceridemia. Mitiglinide stimulates the pancreatic beta cell ATP-sensitive potassium channels, promoting insulin secretion in response to meals. Gemfibrozil acts as an agonist of peroxisome proliferator-activated receptor alpha (PPARα), increasing lipoprotein lipase activity and thereby reducing triglyceride levels. When administered together, gemfibrozil has been shown to significantly increase the bioavailability of mitiglinide due to inhibition of mitiglinide metabolism (notably CYP2C8), which may potentiate both therapeutic and adverse effects. This combination should be used cautiously and requires clinical monitoring, as drug-drug interactions may result in increased risk of hypoglycemia and other side effects, especially relevant in diabetes management[4][2]. There is no evidence that mitiglinide + gemfibrozil is an approved combination product; clinical interest focuses on drug-drug interaction studies rather than therapeutic combination use[2][4].
