**Mitiglinide + metformin** is an oral fixed-dose antidiabetic combination for **type 2 diabetes mellitus**. It combines mitiglinide, a rapid-acting meglitinide insulin secretagogue that binds the sulfonylurea receptor 1 component of pancreatic beta-cell ATP-sensitive potassium channels to promote early-phase, glucose-dependent insulin release, with metformin, a biguanide that primarily reduces hepatic glucose production and improves insulin sensitivity. Fixed-dose tablets containing mitiglinide 10 mg and metformin hydrochloride 500 mg have been studied for pharmacokinetic bioequivalence and clinical glycemic efficacy and are marketed in South Korea as Dia-M. Elixir Pharmaceuticals developed the United States candidate Metgluna, which completed a Phase 3 study in patients inadequately controlled on metformin monotherapy.
