Mitoglitazone (MSDC-0160) is a small molecule insulin sensitizer developed by Metabolic Solutions Development Company for the treatment of type 2 diabetes mellitus. It functions as a selective modulator of the mitochondrial Target of Thiazolidinediones (mTOT), a protein complex in the inner mitochondrial membrane now identified as the Mitochondrial Pyruvate Carrier (MPC). Unlike traditional thiazolidinediones such as pioglitazone, mitoglitazone is designed to be PPAR-sparing, meaning it improves insulin sensitivity and nutrient-sensing pathways without direct activation of the nuclear transcription factor PPARγ. This mechanism is intended to avoid common side effects associated with PPARγ agonists, such as fluid retention and weight gain. In addition to metabolic disorders, mitoglitazone has been investigated for its potential neuroprotective effects in Alzheimer's and Parkinson's diseases.
