Overview

This chemotherapy regimen is a combination treatment studied by Gustave Roussy, Cancer Campus, Grand Paris, specifically for peritoneal carcinomatosis of colorectal or appendiceal origin. The protocol involves the administration of mitomycin C intraperitoneally following surgical tumor resection, paired with systemic administration of fluorouracil (5-FU) and leucovorin calcium. Mitomycin C acts as a potent DNA alkylating agent that induces cross-linking of DNA strands, thereby inhibiting DNA synthesis and function in cancer cells. Fluorouracil is a pyrimidine analog antimetabolite that inhibits thymidylate synthase, an enzyme essential for DNA replication. Leucovorin is included to potentiate the biochemical activity of fluorouracil by stabilizing its binding to thymidylate synthase. This dual-route approach (intraperitoneal and systemic) is designed to maximize local control of peritoneal disease while providing systemic coverage against metastatic spread.
