mitoxantrone + thiotepa + cyclophosphamide is a high-dose chemotherapy combination regimen investigated by the University of Arizona for the treatment of advanced breast cancer. This therapeutic approach, often administered in an outpatient setting, involves dose-escalated mitoxantrone (a type II topoisomerase inhibitor and DNA intercalator), thiotepa (an alkylating agent), and cyclophosphamide (a nitrogen mustard alkylating agent). The regimen was evaluated in a Phase II clinical trial for patients with primary, locally advanced, or stage IV breast cancer. To mitigate the severe toxicity associated with these high doses, the protocol includes the administration of the cytoprotective agent amifostine and is followed by autologous peripheral blood stem cell transplantation (PBSCT) to rescue bone marrow function. The combination aims to maximize tumor cell kill by utilizing multiple mechanisms of DNA damage, including strand breaks, cross-linking, and intercalation.
