Mixed oil lipid emulsion (MOLE) is a multi-component intravenous lipid emulsion used in parenteral nutrition, particularly for extremely preterm infants. Unlike traditional soybean oil-based emulsions, MOLE typically consists of a blend of soybean oil, medium-chain triglycerides (MCT), olive oil, and fish oil. This formulation is designed to optimize the delivery of essential fatty acids while reducing the pro-inflammatory potential associated with high omega-6 intake. The inclusion of fish oil provides omega-3 fatty acids (EPA and DHA), which are critical for neurodevelopment and may mitigate the inflammatory processes leading to bronchopulmonary dysplasia (BPD). The University of Texas Health Science Center, Houston is evaluating MOLE in the LION trial (NCT04602507) to determine its efficacy in improving survival free of BPD and long-term neurodevelopmental outcomes compared to soybean oil-based emulsions.
