Mixed oil lipid emulsion (MOLE) is a multi-component intravenous lipid emulsion used in parenteral nutrition, particularly for extremely preterm infants. It is typically composed of a blend of soybean oil, medium-chain triglycerides (MCT), olive oil, and fish oil. This formulation is designed to provide a balanced profile of essential fatty acids while reducing the high concentrations of omega-6 fatty acids and phytosterols found in traditional soybean oil-based lipid emulsions (SOLE). The inclusion of fish oil provides omega-3 fatty acids (EPA and DHA), which have anti-inflammatory properties and may help prevent parenteral nutrition-associated liver disease (PNALD). In clinical trials like the LION study, MOLE is being evaluated for its potential to reduce the incidence of bronchopulmonary dysplasia (BPD) and improve neurodevelopmental outcomes in neonates by mitigating oxidative stress and inflammation.
