Mixed tocopherols + alpha lipoic acid is an investigational antioxidant combination therapy studied by Vanderbilt University for the treatment of oxidative stress and inflammation in patients with Stage 3-4 chronic kidney disease (CKD). The intervention, evaluated in the Phase 2 TALAT trial, consists of a daily oral regimen of 666 IU of mixed tocopherols (alpha, beta, gamma, and delta isoforms of Vitamin E) and 600 mg of alpha lipoic acid (thioctic acid). Alpha lipoic acid is a naturally occurring thiol-based antioxidant that acts as a cofactor for mitochondrial enzymes and a scavenger of reactive oxygen species, while tocopherols provide lipid-soluble antioxidant protection. Although intended to reduce biomarkers such as F2-isoprostanes and C-reactive protein, the TALAT study found that the combination did not significantly improve these markers compared to placebo in the CKD population.
