MK-0736 is a potent and selective small molecule inhibitor of 11β-hydroxysteroid dehydrogenase type 1 (11β-HSD1), an enzyme involved in the prereceptor metabolism and activation of glucocorticoids within peripheral tissues. By inhibiting 11β-HSD1, MK-0736 was developed to target metabolic disorders such as type 2 diabetes, hypertension, and obesity-related conditions. Clinical studies showed that while MK-0736 was generally well tolerated and produced modest improvements in blood pressure, LDL cholesterol, HDL cholesterol, body weight, and other metabolic parameters in overweight-to-obese hypertensive patients or those with type 2 diabetes mellitus, its efficacy did not meet primary endpoints compared to placebo. As a result of insufficient benefit over standard therapies for these indications, development was discontinued[2][3][4][7][8].
