MK-1092 is a synthetic insulin dimer developed as a novel basal insulin for the treatment of type 1 and type 2 diabetes mellitus. It acts as an insulin receptor partial agonist, designed to provide glycemic control with a reduced risk of hypoglycemia compared to conventional insulins. MK-1092 is part of a new class of antidiabetic agents known as insulin receptor partial agonists (IRPAs). These molecules are dimers of native insulin connected by chemical linkers, resulting in maximal activation levels lower than that of native insulin but with sub-nanomolar potency. Preclinical studies demonstrated efficacy in normalizing plasma glucose while reducing hypoglycemia risk and prolonging duration of action, making it suitable for once-daily dosing. Clinical trials have shown that MK-1092 is well tolerated in healthy adults and patients with both T1DM and T2DM, with dose-proportional pharmacokinetics and pharmacodynamics[3][5][6][8].
