MK-1824 is an investigational, long-acting natriuretic peptide receptor-A (NPR-A) agonist developed by Merck & Co. (known as MSD outside the U.S. and Canada). It is primarily being evaluated for the treatment of heart failure with preserved ejection fraction (HFpEF). NPR-A is a transmembrane receptor that, upon activation by natriuretic peptides like atrial natriuretic peptide (ANP) and brain natriuretic peptide (BNP), catalyzes the production of cyclic guanosine monophosphate (cGMP). This signaling pathway mediates vasodilation, natriuresis, and diuresis, and exerts anti-fibrotic and anti-hypertrophic effects on the myocardium. MK-1824 is designed to provide sustained activation of this pathway to improve hemodynamics and cardiac remodeling in heart failure patients. It is currently in Phase 1 clinical development.
