MK-7680 is a cyclic nucleotide prodrug developed as an oral small molecule. It has been studied primarily for its antiviral activity against Hepatitis C Virus (HCV), with clinical trials conducted in patients infected with HCV genotypes 1 and 3. The drug acts as a substrate for the human renal uptake transporter organic anion transporter 3 (OAT3). Preclinical studies have shown that vehicle choice can significantly affect kidney exposure and toxicity, as polyethylene glycol 200 (PEG 200) inhibits OAT3-mediated uptake of MK-7680 into proximal tubular cells, thereby reducing nephrotoxicity observed in animal models. The precise molecular mechanism of action remains unknown, but it is not considered to have a novel mechanism. The primary indication investigated was Hepatitis C Virus infection; the exact stage of development or approval status is unclear from available sources. Notably, preclinical safety concerns regarding nephrotoxicity were identified and linked to renal transporter interactions modulated by formulation excipients.
