MK-8527 is a novel, orally administered small molecule antiretroviral drug in the class of nucleoside reverse transcriptase translocation inhibitors (NRTTI). It is being developed by Merck for both the prevention (as pre-exposure prophylaxis, PrEP) and treatment of HIV-1 infection. MK-8527 works by inhibiting HIV reverse transcriptase through immediate and delayed chain termination, thereby blocking viral replication. Unlike its predecessor islatravir, MK-8527 has a different chemical structure and so far has not shown the same safety concerns related to lymphocyte depletion. The drug demonstrates potent antiviral activity with a pharmacokinetic profile supporting once-weekly or once-monthly oral dosing. Clinical trials are ongoing to assess its safety, tolerability, and efficacy in both HIV-negative individuals for PrEP and people living with HIV[1][2][4][5][6][8].
