Overview

MK0657 + levodopa is an investigational **combination therapy** consisting of MK0657, a small molecule oral, highly selective antagonist of the NR2B subunit of the N-methyl-D-aspartate (NMDA) receptor, co-administered with levodopa, the standard dopamine precursor for Parkinson's disease. MK0657 aims to modulate glutamatergic transmission implicated in motor disturbances of Parkinson's disease, while levodopa replaces lost dopaminergic tone. In clinical studies, MK0657 as monotherapy or combined with levodopa has not shown additional benefit for levodopa-induced dyskinesias or motor symptoms in Parkinson's patients, achieving target plasma levels but no significant efficacy. The therapeutic rationale is based on animal models showing potential when combining NR2B antagonists with dopaminergic therapy, although this synergy has yet to be confirmed in humans[1][2].
