Overview

MM-151 + istiratumab is a combination of two monoclonal antibody-based biologic drugs, both developed by Merrimack, investigated for the treatment of solid tumors—specifically, certain biomarker-driven subgroups in colorectal cancer and other cancers. MM-151 is an oligoclonal mixture of three fully human monoclonal IgG1 antibodies targeting non-overlapping epitopes on the epidermal growth factor receptor (EGFR), providing comprehensive EGFR inhibition and aiming to overcome resistance to existing anti-EGFR therapies. Istiratumab (MM-141) is a tetravalent bispecific antibody that simultaneously targets and inhibits insulin-like growth factor 1 receptor (IGF-1R) and ErbB3 (HER3) signaling, disrupting tumor pro-survival pathways and resistance mechanisms[1][4][7]. The combination was intended for patients with RAS wild-type colorectal cancer with IGF-1 biomarker expression, based on the strategy of dual EGFR and IGF-1R/ErbB3 blockade to overcome pathways of resistance to EGFR inhibitors[1].
