Overview

Mocetinostat + nivolumab is an investigational combination therapy comprising the class I/IV histone deacetylase inhibitor **mocetinostat** and the anti–PD-1 immune checkpoint inhibitor **nivolumab**. Mocetinostat acts by inhibiting histone deacetylases (including HDAC11), leading to alterations in gene expression that enhance antitumor immune activity and may sensitize tumors to immune checkpoint blockade. Nivolumab blocks PD-1 on T cells, preventing tumor-mediated immune evasion. Preclinical evidence and phase Ib clinical trials indicate that mocetinostat may augment checkpoint inhibitor efficacy by reducing immunosuppressive myeloid cells and regulatory T cell functions, shifting the immune balance toward effector phenotypes. This combination is under clinical investigation, primarily in unresectable metastatic melanoma, and typically evaluated with or without additional inhibitors (e.g., ipilimumab) in these studies[1][2][4].
