Overview

**MOD-246** is a small molecule precision chemotherapy developed by Modifi Biosciences (acquired by Merck) to address temozolomide (TMZ) resistance in glioblastoma (GBM). It targets cancer cells lacking functional O6-methylguanine methyltransferase (MGMT), a DNA repair protein methylated by TMZ, enabling efficacy in MGMT-proficient tumors where standard TMZ fails despite apparent MGMT deficiency. The drug exploits DNA repair pathway vulnerabilities, particularly in GBM, which affects about half of cases with nonfunctional MGMT but shows variable TMZ response.
