Modafinil + escitalopram is a combination of two pharmaceutical agents, modafinil and escitalopram. Modafinil is a wakefulness-promoting agent primarily used to treat excessive daytime sleepiness associated with narcolepsy, obstructive sleep apnea, and shift work disorder. It acts mainly as a dopamine transporter inhibitor, increasing extracellular dopamine levels in the brain[6]. Escitalopram is a selective serotonin reuptake inhibitor (SSRI) commonly prescribed for major depressive disorder and generalized anxiety disorder; it increases synaptic serotonin by inhibiting its reuptake into presynaptic neurons. The combination has been investigated for potential synergistic effects in reducing the subjective and reinforcing effects of cocaine use, based on the hypothesis that concurrent modulation of both dopamine and serotonin systems may be more effective than either agent alone[2][4][5]. However, clinical studies have shown that while modafinil attenuates some positive subjective effects produced by cocaine, adding escitalopram does not significantly enhance this effect[4]. This combination has also been explored as an adjunct therapy in depression to improve remission rates when standard antidepressant therapy is insufficient[3].
