Modakafusp alfa is a first-in-class immunocytokine designed to deliver attenuated interferon-alpha2b specifically to CD38-expressing immune and myeloma cells. It consists of two attenuated interferon-alpha2b molecules fused to an anti-CD38 IgG4 monoclonal antibody, which binds a unique epitope on CD38. This targeted approach activates both innate and adaptive immune responses, leading to direct anti-proliferative and apoptotic effects in myeloma cells. Unlike other anti-CD38 antibodies (such as daratumumab or isatuximab), modakafusp alfa does not rely on direct cytotoxicity but instead delivers an immunologically active cytokine payload, resulting in immune activation and tumor cell death. The drug is being developed primarily for relapsed or refractory multiple myeloma[1][4][5][7][8].
