A lipid-lowering treatment regimen consisting of a moderate-intensity statin (specifically pitavastatin 4 mg or atorvastatin 20 mg) combined with ezetimibe 10 mg. This combination is being investigated by Yonsei University in the ULTRA-CKD trial (NCT05936476) for the secondary prevention of cardiovascular events in patients with chronic kidney disease (CKD) and concomitant atherosclerotic cardiovascular disease (ASCVD). The regimen utilizes two distinct mechanisms: statins inhibit HMG-CoA reductase to reduce hepatic cholesterol synthesis, while ezetimibe targets the Niemann-Pick C1-Like 1 (NPC1L1) receptor to block intestinal cholesterol absorption. This dual-pathway approach is designed to achieve intensive LDL cholesterol lowering while potentially minimizing the muscle-related or metabolic adverse effects associated with high-intensity statin monotherapy.
