Modi-1 is a citrullinated peptide-based cancer vaccine developed by Scancell for the treatment of solid tumors. It consists of three specific peptides derived from vimentin and enolase proteins that are citrullinated and combined with an adjuvant to enhance immune response. The vaccine is designed to stimulate CD4+ T cell responses against tumor cells presenting these neoepitopes via MHC class II molecules. Upon activation at the tumor site, these T cells release proinflammatory cytokines such as interferon gamma (IFNγ), which further upregulates MHC class II expression on tumor cells in a positive feedback loop. The vaccine is administered intradermally using a microneedle device and can be given alone or in combination with checkpoint inhibitors like pembrolizumab. Its primary indications under investigation include head and neck squamous cell carcinoma (SCCHN), triple negative breast cancer (TNBC), renal cell carcinoma (RCC), and high grade serous ovarian carcinoma (HGSOC). Clinical trials are ongoing to assess its safety, immunogenicity, and preliminary efficacy[2][4][6][7].
