Modified Ban-Xia-Xie-Xin Decoction is a Traditional Chinese Medicine (TCM) herbal formula derived from the classic Ban-Xia-Xie-Xin-Tang, originally documented in the *Shanghan Lun* (Treatise on Cold Damage). This modified version, specifically utilized in research by the Hospital of Chengdu University of Traditional Chinese Medicine, is tailored for the treatment of type 2 diabetes mellitus (T2DM) and community-acquired pneumonia (CAP). The formula is designed to address the TCM pattern of "cold-heat complex" (Han Re Cuo Za) in the middle jiao. In the context of T2DM, it is hypothesized to improve insulin sensitivity and glucose metabolism by modulating the gut microbiota and activating the AMPK pathway. For CAP, it is used to regulate the inflammatory response and alleviate respiratory symptoms by inhibiting pro-inflammatory cytokines. The base formula typically consists of *Pinellia ternata*, *Scutellaria baicalensis*, *Coptis chinensis*, *Panax ginseng*, *Glycyrrhiza uralensis*, *Zingiber officinale*, and *Ziziphus jujuba*, with specific modifications (additions or subtractions) depending on the clinical presentation.
