Overview

Modified BUCY is an intensified myeloablative conditioning (MAC) regimen primarily utilized in allogeneic hematopoietic stem cell transplantation (allo-HSCT) for patients with high-risk acute myeloid leukemia (AML). Developed and extensively studied by the First Affiliated Hospital of Soochow University, this regimen modifies the standard Busulfan and Cyclophosphamide (BuCy) protocol by incorporating additional cytotoxic agents, typically cytarabine (Ara-C) and etoposide (VP-16). The combination aims to maximize the anti-leukemic effect through multiple mechanisms: busulfan and cyclophosphamide act as DNA alkylating agents, cytarabine functions as an antimetabolite inhibiting DNA polymerase, and etoposide inhibits topoisomerase II. This multi-drug approach is designed to reduce the incidence of relapse in high-risk populations while maintaining a manageable safety profile during the transplant process.
