mBU + CY + ATG is a conditioning regimen used as a preparative treatment for patients with acute leukemia undergoing allogeneic hematopoietic stem cell transplantation (allo-HSCT), particularly haploidentical transplants. Developed at Peking University People's Hospital and often referred to as part of the "Beijing Protocol," this regimen is designed to provide potent myeloablation and anti-leukemic activity while managing the risk of graft-versus-host disease (GVHD). The "modified busulfan" (mBU) component typically includes busulfan combined with cytarabine (Ara-C) and semustine (Me-CCNU). Cyclophosphamide (CY) acts as an additional alkylating agent, and anti-thymocyte globulin (ATG) is included to facilitate engraftment and prevent GVHD by depleting host and donor T-cells. Recent clinical trials have also evaluated the addition of decitabine to this regimen to further reduce relapse rates in refractory or relapsed acute leukemia.
