Overview

Modified Early Post-Operative Intraperitoneal Chemotherapy (mEPIC) is a chemotherapy regimen designed to maximize therapeutic benefit and reduce logistical barriers compared to standard Early Post-Operative Intraperitoneal Chemotherapy (EPIC). The regimen shortens the postoperative intraperitoneal chemotherapy from five to two days. It involves administering one intraperitoneal bolus of fluorouracil (5-FU) at 400 mg/m2, followed by a 24-hour intraperitoneal infusion of 5-FU at 1200 mg/m2, on postoperative days 1 and 2. This approach aims to exploit the known pharmacokinetics and pharmacodynamics of 5-FU, which primarily acts by inhibiting thymidylate synthase, while improving feasibility and patient management. It is being investigated for the treatment of peritoneal carcinomatosis arising from colorectal and appendicular neoplasms.
