Overview

Modified Er Zhi Wan (二至丸加味) is a traditional Chinese medicine (TCM) herbal formula based on the classic Er Zhi Wan ("Two-Solstice Pill"), which consists of two main herbs—Glossy Privet Fruit (Nu Zhen Zi) and Eclipta Herb (Mo Han Lian)—and is used to nourish and tonify Liver and Kidney Yin. The "modified" version refers to the addition of other herbs tailored to specific patient presentations or disease patterns according to TCM principles. The core mechanism involves replenishing Yin in the Liver and Kidneys, cooling the blood, stopping bleeding, and addressing symptoms such as dizziness, premature graying of hair, weakness in lower back/knees, insomnia due to Yin deficiency heat. Modifications may target additional symptoms or comorbidities by including herbs for calming the mind (e.g., Suan Zao Ren), invigorating blood circulation (e.g., Dan Shen), or further nourishing Yin. In Western medical terms, it is most often used for conditions associated with chronic fatigue syndromes, menopausal symptoms, abnormal uterine bleeding due to hormonal imbalance or perimenopause/menopause transition states with underlying "Yin deficiency," as well as adjunctive therapy for certain kidney diseases like IgA nephropathy[1][2][3][6].
