Overview
modified FLOT or modified FOLFOX-6 refers to the induction chemotherapy regimens utilized in the PULSO trial (NCT06906887), sponsored by the Medical College of Wisconsin. This protocol is used for patients with Stage II-IVb esophageal adenocarcinoma. FLOT is a four-drug regimen consisting of 5-fluorouracil, leucovorin, oxaliplatin, and docetaxel, while FOLFOX-6 is a three-drug regimen consisting of 5-fluorouracil, leucovorin, and oxaliplatin. These regimens function through multiple mechanisms, including the inhibition of thymidylate synthase by 5-fluorouracil, DNA cross-linking by oxaliplatin, and microtubule stabilization by docetaxel. In the context of the PULSO trial, these are administered as induction therapy for 2-4 months prior to randomized chemoradiation (Pulsed Low-Dose-Rate vs. standard radiation) to evaluate the reduction of esophagitis.
