Modified FOLFOX6 (mFOLFOX6) is a combination chemotherapy regimen consisting of oxaliplatin, leucovorin (folinic acid), and 5-fluorouracil (5-FU). It is a standard-of-care treatment for colorectal cancer and is frequently utilized in the treatment of other gastrointestinal malignancies, such as gastric and pancreatic cancers. The 'modified' designation typically refers to the omission of the bolus 5-fluorouracil injection or a reduction in the oxaliplatin dose to mitigate side effects like neutropenia and peripheral neuropathy. The regimen's efficacy stems from the synergistic action of its components: oxaliplatin creates DNA cross-links that inhibit replication, while 5-FU acts as an antimetabolite that inhibits thymidylate synthase, an effect that is biochemically enhanced by the addition of leucovorin.
