Modified gp100 peptide-loaded dendritic cells are an investigational cellular immunotherapy in which patients' dendritic cells are isolated, matured, and ex vivo loaded (pulsed) with gp100 peptides that have been modified (typically at specific amino acid positions) to improve binding to MHC class I molecules and potentially enhance T cell immunogenicity. Upon administration, these dendritic cells present the modified gp100 peptide antigen—derived from the melanoma-associated glycoprotein 100 (gp100)—to cytotoxic T lymphocytes, aiming to stimulate a tumor-specific immune response. This therapy has been primarily evaluated in advanced HLA-A2.1-positive melanoma, with administration by intravenous and intradermal injection. Clinical studies show the approach can induce long-term responses in a minority of patients, with no clear evidence that modified peptides outperform wild-type peptides in vivo for inducing immune or clinical responses.
