Overview

Modified medium-chain triglyceride refers to structurally or compositionally altered medium-chain triglycerides (MCTs), which are triglycerides containing 6–12 carbon fatty acid chains. MCTs, whether modified or not, are rapidly absorbed and metabolized by the body to provide a rapid energy source and are less dependent on bile salts for absorption compared to long-chain triglycerides. Modified versions may be produced to optimize properties such as fatty acid composition for specific clinical applications, stability, or absorption profile. MCTs are used mainly in the dietary management of disorders with fat malabsorption (e.g., steatorrhea, pancreatic insufficiency, short bowel syndrome) and also in ketogenic diets (notably for epilepsy) due to their ability to generate ketones. Proposed mechanisms include rapid hydrolysis to medium-chain fatty acids and subsequent β-oxidation, bypassing typical chylomicron transport and promoting metabolic effects such as increased energy expenditure, improved satiety, and potential modulation of gene expression related to metabolism, possibly via pathways involving AMPK and Akt signaling, and inhibition of TGF-β signaling[2][1][9]. Modified forms may be designed to further improve tolerability, absorption, physical properties, or specificity for use in parenteral or enteral nutrition.
