Modified non-clarithromycin triple therapy refers to a combination regimen for the eradication of Helicobacter pylori infection that includes a proton pump inhibitor (PPI) and two antibiotics, but specifically excludes clarithromycin due to rising resistance rates. The most common modifications substitute clarithromycin with other antibiotics such as metronidazole, levofloxacin, or rifabutin. These regimens are used as alternatives in regions or patient populations where clarithromycin resistance is high or when prior treatment with clarithromycin has failed. The mechanism of action involves suppression of gastric acid secretion by the PPI (enhancing antibiotic efficacy) and direct antibacterial effects from the two chosen antibiotics against H. pylori[1][3][5]. This approach is recommended in recent guidelines as an initial or salvage treatment for H. pylori infection.
