Modified Ponticelli regimen

The Modified Ponticelli regimen is a cyclical immunosuppressive protocol used primarily for the treatment of idiopathic membranous nephropathy (IMN). It is an adaptation of the original Ponticelli regimen, substituting the alkylating agent chlorambucil with cyclophosphamide to reduce toxicity while maintaining efficacy. The regimen typically spans six months, alternating monthly cycles of corticosteroids (intravenous methylprednisolone pulses followed by oral prednisolone) and oral cyclophosphamide. This combination aims to induce remission of nephrotic syndrome by broadly suppressing B-cell and T-cell activity and reducing glomerular inflammation.
