Overview

Modified RNA (modRNA) influenza vaccines are a class of messenger RNA-based prophylactics that utilize nucleoside-modified mRNA to encode viral antigens, primarily the hemagglutinin (HA) protein of influenza viruses. By incorporating modified nucleosides such as N1-methylpseudouridine, these vaccines minimize innate immune activation and maximize the translation of the encoded antigen within host cells. The mRNA is typically encapsulated in lipid nanoparticles (LNPs) to facilitate delivery and protect the genetic material from degradation. A prominent example is the quadrivalent modRNA vaccine developed by Pfizer and BioNTech (PF-07252225/BNT161), which encodes HA antigens from four WHO-recommended influenza strains (two A and two B). In Phase 3 clinical trials, this candidate demonstrated superior relative efficacy against laboratory-confirmed influenza-like illness compared to standard inactivated vaccines, although it was associated with increased reactogenicity.
