Modified Simiao San is a traditional Chinese medicine (TCM) herbal formula derived from the classical Simiao San (Four Marvels Powder), which was originally recorded by Sun Simiao during the Tang Dynasty. The "modified" version typically includes additional herbs to enhance its therapeutic effects for specific indications such as osteoarthritis and gouty arthritis. Modified Simiao San is primarily used for its anti-inflammatory and urate-lowering properties. Mechanistically, it has been shown to regulate macrophage polarization (M1/M2) via inhibition of the NF-κB signaling pathway in osteoarthritis models[1], reduce serum uric acid levels by modulating renal transporters involved in uric acid excretion[3][4], and promote glucose disposal through AMP-kinase regulation in adipocytes[5]. Clinical studies have demonstrated efficacy in reducing symptoms of gouty arthritis and lowering inflammatory markers such as C-reactive protein[3][4]. It is generally considered safe when used appropriately.
