Modified Tian Wang Bu Xin Dan is a pharmacological adaptation of the classical Traditional Chinese Medicine (TCM) formula Tian Wang Bu Xin Dan (TWBXD), which was originally documented in 1337 for the treatment of insomnia, anxiety, and palpitations. This modified version involves specific alterations to the herbal composition and is being investigated in novel delivery formats, such as a transdermal hydrogel patch, to prevent postoperative delirium (POD) in elderly patients. The original formula typically contains 13 herbal ingredients (including *Rehmannia glutinosa*, *Ziziphus jujuba*, and *Panax ginseng*) that nourish the heart-yin and calm the spirit. The modified preparation is hypothesized to stabilize the mind and reduce cognitive fluctuations by modulating the hypothalamic-pituitary-adrenal (HPA) axis, reducing systemic cortisol levels, and enhancing central GABAergic and serotonergic signaling. It has shown potential clinical efficacy comparable to antipsychotics like olanzapine in managing delirium symptoms.
