Overview

Modified TPF (mTPF) is a chemotherapy regimen consisting of lower or adjusted doses of three cytotoxic agents—docetaxel, cisplatin, and fluorouracil (5-FU)—compared to the standard TPF protocol. It is primarily used as induction chemotherapy for patients with locally advanced head and neck squamous cell carcinoma (LAHNSCC), especially those who are frail or have significant comorbidities that preclude standard dosing. The mechanism of action involves the inhibition of cell division and DNA synthesis in rapidly dividing cancer cells through multiple pathways: - Docetaxel stabilizes microtubules, preventing their depolymerization and thereby inhibiting mitosis. - Cisplatin forms DNA crosslinks leading to apoptosis. - Fluorouracil inhibits thymidylate synthase, disrupting DNA synthesis. Modified TPF regimens have demonstrated similar efficacy but improved tolerability compared to standard TPF in certain populations[1][5]. The regimen is typically administered intravenously during induction therapy or concurrently with radiotherapy.
