This combination drug consists of the monoacylglycerol (MAG) forms of the omega-3 fatty acids eicosapentaenoic acid (EPA) and docosahexaenoic acid (DHA). Developed primarily to address fat malabsorption, these monoglycerides are unique because they are absorbed directly by the intestinal mucosa without requiring hydrolysis by pancreatic lipase or the presence of bile salts. This "pre-digested" formulation is particularly beneficial for patients with pancreatic insufficiency, such as those with cystic fibrosis or chronic pancreatitis. Once absorbed, the EPA and DHA components exert anti-inflammatory and lipid-modulating effects by acting as agonists for GPR120 and PPAR-alpha, while also competitively inhibiting the cyclooxygenase (COX) and lipoxygenase (LOX) pathways to reduce the production of pro-inflammatory eicosanoids.
