The monovalent GBS-CRM glycoconjugate vaccine is a candidate vaccine designed to prevent infections caused by Group B Streptococcus (GBS), specifically targeting a single serotype (such as type V). It consists of the capsular polysaccharide (CPS) from the targeted GBS serotype covalently linked to CRM197, a nontoxic mutant of diphtheria toxin used as a carrier protein. This conjugation transforms the otherwise T-cell independent bacterial polysaccharide into an antigen capable of eliciting robust T-cell dependent immune responses, including memory and high-affinity IgG production. The mechanism involves uptake by antigen-presenting cells, processing into glycan-peptide fragments that are presented via MHCII molecules to carbohydrate-specific CD4+ T cells, which then provide help for B cell antibody production[1][6][8]. The primary indication is prevention of invasive GBS disease in at-risk populations such as pregnant women and neonates. Monovalent formulations have been tested in phase I/II trials and shown improved immunogenicity over unconjugated polysaccharides[1].
