Montanide ISA-51 is a vaccine adjuvant used to enhance immune responses in therapeutic and prophylactic vaccines, particularly in cancer and infectious disease trials. It is a water-in-oil emulsion composed of mineral oil and the surfactant mannide monooleate, with the "VG" version containing vegetable-grade oleic acid derived from olive oil[1][2][3]. Its mechanism of action involves forming an antigen depot at the injection site, supporting slow release of antigens and enhancing cytotoxic T lymphocyte (CTL) responses as well as CD4+ and CD8+ T cell activation[1][6][8]. Montanide ISA-51 has been widely used in clinical trials for cancer (notably melanoma, lung cancer), AIDS/HIV vaccines, glioblastoma, gliosarcoma, malaria, and autoimmune diseases[3][5].
