Overview

Montelukast + pranlukast is a combination of two small molecule leukotriene receptor antagonists, both of which selectively block the cysteinyl leukotriene type 1 receptor. These drugs are used primarily for the prophylactic treatment and management of asthma and allergic rhinitis. By inhibiting the action of leukotrienes, they reduce bronchospasm and inflammation triggered by allergens or other stimuli in susceptible individuals. Montelukast is widely used globally, while pranlukast is mainly available in Japan and South America. Both agents have similar pharmacodynamic profiles but differ in potency and pharmacokinetics. Adverse effects include gastrointestinal disturbances and neuropsychiatric events such as insomnia, agitation, anxiety, depression, tremor, irritability, headache, somnolence, diarrhea, dizziness; these are more frequently reported among women aged 19–64 years[1][3][8]. The combination has not been established as a marketed fixed-dose product but may be considered theoretically for additive or synergistic effects on leukotriene-mediated pathways.
