Overview

Montelukast + salmeterol is a combination of two pharmaceutical agents used in the management of asthma and other obstructive airway diseases. Montelukast is an oral leukotriene receptor antagonist that blocks the action of leukotriene D4 on the cysteinyl leukotriene receptor CysLT1 in the lungs, leading to decreased inflammation and bronchoconstriction. Salmeterol is an inhaled long-acting beta-2 adrenergic receptor agonist (LABA) that causes prolonged bronchodilation by stimulating beta-2 receptors on bronchial smooth muscle. While both drugs are commonly used as adjuncts to inhaled corticosteroids for asthma control, their combination (without a corticosteroid) is not standard practice but has been studied for additive or synergistic effects on bronchodilation and protection against exercise-induced or cold-air-induced bronchospasm[1][2]. The combination may provide greater bronchodilatation than montelukast alone, but not more than salmeterol alone; however, their protective effects against airway obstruction appear additive[2].
