Montelukast sodium + levocetirizine is a fixed-dose combination (FDC) therapy comprising a leukotriene receptor antagonist (LTRA) and a second-generation H1-antihistamine. Montelukast sodium selectively antagonizes the cysteinyl leukotriene receptor 1 (CysLT1), which is found in the human airway and on pro-inflammatory cells, thereby reducing bronchoconstriction, mucous secretion, and vascular permeability. Levocetirizine, the active R-enantiomer of cetirizine, acts as a potent and selective peripheral H1-receptor antagonist, providing relief from histamine-mediated allergic symptoms such as sneezing, rhinorrhea, and pruritus. This combination is widely utilized for the management of allergic rhinitis, particularly when accompanied by asthma, as it addresses both the early-phase (histamine-driven) and late-phase (leukotriene-driven) allergic responses. It is typically administered orally once daily.
