morinidazole

Morinidazole is a novel 5-nitroimidazole-class antibacterial drug primarily indicated for the treatment of infections caused by anaerobic bacteria and certain protozoa. It is particularly effective against pathogens such as Bacteroides fragilis, Clostridium species, amoeba, and Trichomonas vaginalis. Morinidazole acts via nitroreduction within susceptible microorganisms, leading to DNA damage and cell death. Compared to older nitroimidazoles like metronidazole and ornidazole, morinidazole demonstrates higher in vitro antibacterial activity against anaerobes and has improved safety characteristics due to its high water solubility (which limits central nervous system penetration) and metabolism that does not involve the CYP system—resulting in fewer drug interactions and less impact on liver function. The China Food and Drug Administration approved morinidazole in 2014 for clinical use[1][3][6]. Morinidazole is structurally related to metronidazole but offers advantages such as reduced neurotoxicity risk due to lower blood–brain barrier penetration, minimal CYP-mediated metabolism (reducing potential for drug-drug interactions), and greater efficacy against some resistant anaerobic strains[3][6]. It has been studied both alone and in combination with other antibiotics (e.g., levofloxacin) for pelvic inflammatory disease.
