The study investigated whether the timing of thiopurine administration (azathioprine or 6-mercaptopurine) influences metabolite levels and clinical outcomes in patients with inflammatory bowel disease (IBD), specifically Crohn's disease and ulcerative colitis. Thiopurines are small molecule immunomodulators that function as purine antimetabolites; they are metabolized into active 6-thioguanine nucleotides (6-TGN), which incorporate into DNA and inhibit Rac1, thereby inducing apoptosis in activated T lymphocytes. The Morning Group intervention required patients to take their medication between 6:00 am and 11:00 am. The trial's findings suggested that morning dosing may optimize the metabolic profile by increasing therapeutic 6-TGN levels and decreasing potentially hepatotoxic 6-methylmercaptopurine (6-MMPR) levels compared to evening dosing.
